What is required for your consultation at Mid-South DPC ?
All patients must register a new patient chart by clicking this link New Patient Enrollment , you will be asked to select a membership based on age but you will not be charged a membership fee.
2. Previous medical records documenting qualifying condition are required and must be reviewed prior to having an appointment scheduled. These can be brought by the clinic in person, emailed or faxed.
3. Once records have been reviewed and approved for scheduling the patient will be given an appointment, we often have same week appointments available.

What conditions can be treated with a medical card?
Only patients who have registered with the Mississippi Department of Health and have received written certification from a certifying provider can be treated.
The State of Mississippi has determined that the following diseases or conditions qualify
Cancer
Parkinson's disease
Huntington's disease
muscular dystrophy
glaucoma
spastic quadriplegia
positive status for human immunodeficiency virus (HIV)
acquired immune deficiency syndrome (AIDS)
hepatitis
amyotrophic lateral sclerosis (ALS)
Crohn's disease
ulcerative colitis
sickle-cell anemia
Alzheimer's disease
agitation of dementia
post-traumatic stress disorder (PTSD)
autism
pain refractory to appropriate opioid management
diabetic/peripheral neuropathy
spinal cord disease or severe injury
Also qualifying is a chronic terminal or debilitating disease or medical condition or its treatment that produces one or more of the following:
cachexia or wasting syndrome
chronic pain
severe or intractable nausea
seizures
severe and persistent muscle spasms including, but not limited to, those characteristic of multiple sclerosis
How can I get a medical card in the State of Mississippi?
Step 1
Register as a patient with the Mississippi State Department of Health (MSDH). Registration is all online and will take only a few minutes. After patients have completed the online registration, they will receive verification along with their MSDH patient ID number. For more information about the process, go to MSDH’s website at

Step 2
Obtain a written certification from a provider registered with the MSDH. Once registered, patients must obtain a valid written certification from a provider registered with the MSDH, such as Dr. Hendry or David Snodgrass. After a complete and thorough evaluation, your provider can approve the medical card as a treatment option if they determine a patient has a qualifying medical condition as listed above.
How much does it cost to get a medical card in the State of Mississippi?
Registering as a patient with the MSDOH is $25.
Initial 30 minute visit and certification at Mid-South DPC with qualifying condition is $175 , This includes the state required 6 month follow up visit for FREE which can be done via tele-health or in person.
After first year of being a patient yearly re-certification visits are $150 and can be done via tele-health or in person
